The hip joint consists of the femoral head and an acetabulum, both lined with articular cartilage that allows the bones to move within the joint with less friction.

The socket area of the acetabulum is covered with tough cartilage called labrum. These areas can wear (osteoarthritis) away and become the source of pain.

The main symptoms of osteoarthritis of the hip are:

  • Hip pain that persists beyond a few days
  • Inability to walk comfortably on the affected side
  • Pain is more gradual in onset and does not respond to rest
  • Hip pain occurs at night or while resting
  • Hip pain limits the range of motion or causes limping.

Osteoarthritis is the most common cause of hip pain.  Approximately 80% of the population older than 70 years has osteoarthritis, not everyone has complaints there.

Treatment

Without surgery

If the osteoarthritis is not yet at an advanced stage, provide the following treatments will help:

  • Restrict overweight
  • Strengthen of the quadriceps (possibly supported by physical therapy).
  • Analgesics in combination with anti-inflammatory medication.
  • Adjustment of shoes or using a cane
  • Anti-inflammatory injection in the hip joint

If there is uncertainty about the origin of your pain and possibly the result might come back, sometimes a painkilling injection is given in the hip joint.
 
With surgery

Hip osteoarthritis in an advanced stage can be treated as follows:

  • Arthroscopy; not commonly used to evaluate hip joint damage because it is technically difficult to perform and includes a high risk of complications.
  • Osteotomy. The hip bone is cut and attached in another position. It is an appropriate operation for younger people with osteoarthritis if the shape of the hip differs. The osteotomy moves the body load area to a healthier part of the femoral head.
  • Arthrodesis. An arthrodesis fix the hip joint.  The hip joint is not functional anymoreThis treatment is very unusual.
  • "Surgical hip dislocation and nettoyage". This is a major operation, sporadically used in young patients. The hip joint is cleaned by dislocation both joint parts and bone proliferations are removed.
  • Hip replacement is perhaps the most common treatment. It is considered in patients with progressive arthritis that affects the patient's ability to do daily activities.

Questions

Is hip arthritis associated with osteoporosis?
No, these are different diseases. Osteoporosis has to do with low bone density, while osteoarthritis is related to cartilage damage in joints.

Is my overweight associated with my arthritis?
Obesity is not clearly associated with osteoarthritis of the hip. Weight loss can reduce the symptoms and reduce the risk of any subsequent surgery with a healthy weight.

Am I eligible for cartilage transplant?
No. Cartilage transplant is (still) not applicable in the hip joint.

Is it too late for an operation with a worn out hip?
Yes, if the wear is too far. Your orthopedic surgeon will discuss with you the options.

Will my hip joint worn out faster if I take painkillers?
For osteoarthritis in the hip it is wise to adjust the load on your affected side. Using a tranquilizer does not accelerate the wear, but it delays the need of surgery. Especially, if you are young of age, it may be advisable.
